How we work with health plans
We offer a suite of care coordination and clinical services, helping health plans engage and enroll their members in ongoing support programs and primary care services. We’re here to make sure your members get the comprehensive care they deserve.
Our services
We offer Enhanced Care Management (ECM), community supports, and a community health worker benefit.
To increase access and availability, we coordinate with member's existing providers, or match members with Zócalo Health providers based on personal preferences, to offer virtual primary and urgent care services for immediate needs.
We provide clinical and mental health assessments, therapy, support groups, and family counseling.
Our promotoras de salud help members enroll and access community supports and social benefits programs such as meal delivery, housing, legal aid, and childcare.
Our local outreach specialists and promotoras de salud, find, engage, and build trust with members that are the hardest to reach for longer-term relationships and outcomes.
We work with members both in-person and virtually to provide complex care management. We also coordinate their primary and social needs before and after clinical appointments, ensuring continuity of care.
Our care team supports in proactively identifying and addressing unmet healthcare needs, ensuring members receive essential preventive, chronic, and follow-up care.
Providing integrated care
Maria, who lives in Merced County, first learned about Zócalo Health when a local outreach specialist contacted her. Her promotora de salud, Alondra, quickly became a trusted presence in her life, enrolling her in ECM program benefits and helping her navigate challenges she’d faced for years.
“It’s like we’re a team,” says Maria. “Whatever she can do on her end, she’ll go ahead and do it.”
Overcoming health barriers
Maria had long struggled with asthma, but with her PCP’s long wait times, she found it difficult to access timely care. Alondra stepped in to connect Maria with Dr. Nona, the MD on her care team, who facilitated a same-day virtual appointment and ensured that her inhaler prescription was available at a local pharmacy that evening.
Providing mental health support
Juggling work and family life had taken a toll on Maria’s mental health. Recognizing her need for support, Alondra made monthly check-ins a part of their routine.
“Some people just need someone to just ask them how they're doing,” says Maria. “She knows I’m a mom, and she knows I work a lot. It can be a lot to take on. She says, ‘If you need to talk, there are therapists.’ I like that she calls once a month to check in.”
Addressing everyday struggles
Maria faced housing instability and mounting utility bills, but Alondra helped her navigate solutions she didn’t know were available. For example, when an overdue PG&E bill became unmanageable, Alondra connected her with resources that forgave the balance entirely. She also arranged weekly food deliveries and helped with other essentials like diapers and toys.
“I feel like she has come into our family,” says Maria. “It’s like we’re not alone, like we have someone we can count on.”
