Supportive Care for Eligible DaVita Dialysis Members

At Zócalo Health, we are proud to partner with DaVita to bring culturally centered, whole-person Enhanced Care Management (ECM) services to eligible DaVita members living with complex medical and social needs. Our goal is to make sure every eligible DaVita dialysis patient receives coordinated, compassionate support, whether they’re at the dialysis center, at home, or in their community.

What Is Enhanced Care Management (ECM)?

ECM is a no-cost, high-touch care management benefit offered to eligible members who need additional support navigating their health, treatment, and daily challenges. ECM connects members with a dedicated Community Health Worker (CHW) who becomes their main point of support, helping them manage medical needs, coordinate care, access resources, and achieve personal health goals.

Who Qualifies Through Our DaVita Partnership?

DaVita’s eligible members receiving dialysis at participating DaVita centers may qualify for ECM if they experience any of the following:

  • Chronic and complex medical conditions
  • Difficulty coordinating care or keeping up with appointments
  • Barriers such as transportation, food insecurity, housing instability, or lack of social support
  • Challenges managing medications or treatment plans
  • Frequent hospital or emergency room visits

Eligibility is confirmed in collaboration with DaVita social workers and the Zócalo team.

How ECM Supports DaVita Eligible Patients

Every enrolled eligible patient is paired with a bilingual/bicultural Community Health Worker who walks alongside them throughout their care journey. Our CHWs provide:

Personalized Care Coordination

  • Support scheduling and preparing for primary care provider, specialist, and behavioral health visits
  • Assistance understanding treatment plans, labs, and provider recommendations presumably, not limited to dialysis
  • Help navigating insurance or benefit questions

Ongoing Check-Ins & Goal Setting

  • In-person or virtual visits
  • Collaborative care plans based on the patient’s goals and priorities
  • Monitoring progress and adjusting support as needs change

Help with Social & Daily-Life Needs

  • Transportation assistance
  • Access to food, housing, or financial resources
  • Support applying for community programs (CalFresh, utility discounts, etc.)
  • Connection to mental health and wellness services

Support After Hospital or ER Visits

  • Timely post-discharge follow-up
  • Coordination of medications, appointments, and recovery needs

Hello, my name is Erika, and I am a Community Health Worker at Zocalo. I was born in Mexico and raised in Santa Ana, California. As the eldest daughter in my family and a proud first-generation Latina, I am honored to be the first in my family to earn a college degree. With a background in Psychology and Counseling, I am equipped with the skills necessary to support families as they navigate healthcare systems and access social services. My passion lies in empowering others, creating positive change within my community, and advocating for those who need it most. Outside of work, I cherish spending time with my family, whether it's camping, hiking, or exploring new places together.

Hi, I’m Rosaicela, a bilingual Community Health Worker dedicated to helping Latino families across the Inland Empire access the care and resources they need to thrive. I have led vaccine equity initiatives, supported members through Enhanced Care Management, and connected individuals with essential services such as transportation, housing, food, and primary care, always with compassion, cultural understanding, and a deep commitment to community well-being. Outside of work, I enjoy practicing yoga, leading group exercise classes, traveling, exploring nature, and spending quality time with my family.
