How we work
Boots on the ground
Our local outreach specialists find and engage members through individual outreach, community-based events, and partnerships with businesses and institutions like churches and schools. We connect with complex, high-risk patients in their own communities — because they’re our communities too.
Comprehensive care
Every member has a dedicated care team with an in-person promotora de salud (CHW), a virtual MD, and a virtual behavioral health specialist. Following intake and assessment, their promotora facilitates development of a care plan tailored to their unique medical, social, and mental health challenges.
Lasting connections
Our promotoras work closely with members to coordinate their care, helping with doctor’s appointments, community support services, and more. With care teams from the same background as members and practices shaped by Latino traditions and values, we build trusted, lasting relationships.

As children, our founders, Erik Cardenas and Mariza Hardin, each witnessed the lack of access to quality health care in their comunidades — and it inspired them to build a better way. A way that’s by Latinos, for Latinos. A way that tells our stories and celebrates our culture. A way that combines spiritual practices with modern medicine and the technology of today.
